We meet the special needs of subacute and skilled level patients with physical, occupational, speech, and medical nutritional therapies, combined with 24-hour nursing care. Whether provided for several weeks or several months, our physician directed subacute and skilled health care services are designed to complement a hospital stay or, in many cases, be a substitute for hospitalization.
Subacute care is the comprehensive, inpatient care designed for someone who has an acute illness, injury, or exacerbation of a disease process. It is a goal-oriented treatment rendered immediately after, or instead of, acute hospitalization to treat one or more specific active complex medical conditions or to administer one or more technically complex treatments, in the context of a person's underlying long-term conditions and overall situation.
At NHC HealthCare, North Augusta, an interdisciplinary approach is used to formulate a goal-oriented plan of care for each patient. Our subacute care team of doctors, therapists, nurses and other health professionals are dedicated to deliver a quality experience and good results to all concerned. Our focus is work with the patient and their family to assure the most efficient care to achieve their highest level of independence as quickly as possible.
